Job Description & Responsibilities

The Home Health Physical Therapist helps patients regain mobility, manage pain, and improve strength and balance following an injury, surgery, or chronic illness. Working directly in the patient's home, the PT evaluates the living environment for safety and designs customized rehabilitation plans to promote independence.

  • Conduct initial assessments of patients' mobility, strength, balance, and joint range of motion.

  • Develop and implement individualized physical therapy plans of care.

  • Document patient progress and collaborate with physicians and the broader care team.

Certifications & Qualifications
  • Education: Master’s or Doctorate degree in Occupational Therapy.

  • Licensure: Active State Occupational Therapist License.

  • Certification: National Board for Certification in Occupational Therapy (NBCOT) certification; BLS/CPR Certification.

  • Other: Valid driver’s license and reliable transportation.

Job Description & Responsibilities

The Home Health Aide provides essential, non-clinical personal care and routine daily support to patients who are elderly, disabled, or recovering from illness. They focus on maintaining the patient's comfort, hygiene, and emotional well-being.

  • Assist with personal hygiene, including bathing, grooming, and toileting.​

  • Prepare meals according to dietary guidelines and assist with feeding if necessary.

  • Perform light housekeeping, laundry, and grocery shopping.

Certifications & Qualifications
  • Education: High school diploma or equivalent.

  • Certification: Home Health Aide (HHA) Certificate (requires completion of a state-approved training program, this is included in job training)

  • Certifications: CPR and First Aid Certification.

  • Other: Background check clearance.
